- Kickoff: introduce CBO to PS team, ask them questions to better understand their community / services, and set up subsequent meetings (training and quarterly check ins)

- Training: basic meet n’ greet and training on screening form for anyone who will be submitting screenings

How long is the ECM Screening Program contract in effect after the effective date on the contract? What does that mean

it’s perpetual. This means the partnership doesn’t end unless one of the parties chooses to end it.

Can Pair Team enroll people into Medi-Cal if they don’t have coverage yet?

No. We will give them a call and point them towards resources to sign up for Medi-Cal and provide them with our callback number, but we highly recommend that you work with them to ensure they become enrolled.

How often are the recurring check in meetings with ECM Screening Program partners?

usually quarterly, but the PS team is always available via email/ phone/ jumping on zoom

Why does the contract mention details about CHW Enablement if a CBO is only contracting for ECM Screening Program?

We have a standard services agreement for our CBO partners. The agreement lists various services that the CBO and Pair Team will include as part of our partnership. The CHW Enablement services are listed, but will not have a “checkmark” next to them on ECM Screening Program contracts.

What factors lead to a higher conversion rate for a CBO's screenings > ECM enrollments? 

size of CBO -- more volume means lower conversion rate

Why are our virtual services effective? How do we know that patients prefer virtual care?

- Services are effective because they address certain barriers (time, transportation, motivation) which are caused by in-person meetings. We have gold-standard rates of ED reduction, uniting folks with healthcare, etc. 

- We know patients prefer this because it’s hard to get them to come to in-person appointments. Currently less than 10% of our visits are in-person.

It sounds like you’ll be paying us for referrals, which isn’t legal. Can you explain?

- These are screenings not referrals, a screening simply describes someone’s condition and circumstances. Pair Team ultimately determines if a person is eligible before contacting them

- Reference larger picture of VBE: We operate within a relatively new legal framework which allows us to pay our partners for activities such as screenings and care coordination
