What info do you need to put for ID notes when scheduling a meeting?
Date and time of meeting, where the meeting will take place and what type of meeting it is.
Ex: Annual meeting was scheduled to take place in family home on 7/25 at 3:30pm
What is a requirement for a client to receive SLS?
Name on lease/deed, 18 years or older
What are the three goals that Tailored Day Services (TDS) can work on?
Education, Employment or Community Outings
What information needs to be put in the notes for company mileage?
Name of client, type of meeting and time of meeting

What is the process to refer to ILS or SLS?
Fill out 653, do 653 checklist, referral packet, send to manager the once you get the recommendations, you send out the first 2 pages of the 653 referral with the packet.
What are the requirements for daycare services?
When family is working or at school and no one is able to take care of client. Can be someone that lives in the home but cannot be a parent.
Example: Parent is working or going to school 8-6 and client goes to program until 3. We can provide day care from 3-6.
What is the purpose of the CMS/HCBS addendums and when is the deadline?
To make sure clients know they have options in regards to the program and the deadline is end of August
What are the names of all the adult and residential managers?
Adrian, Adrianna, Bridget, Gina, Jenni, Lezette, Olivia, Scott and as of 8/12 Armando
