The employee’s Primary Care Provider (PCP) (remote hires advised likewise)
If you receive immunization documentation that is in another language, what should you do?
Verify it has two patient identifiers, review using google translate or similar product and document use of tool, review with a colleague if its questionable.
Of Rubella, Rubeola, and Mumps, which disease only requires one dose of vaccine to be considered compliant?

The employee states, "I just got my shingles vaccine, does that count?"
If the individual has documentation of 1-2 Hepatitis B vaccine dose(s) and the vaccine brand/manufacturer is unknown, what are your next steps?
Instruct the individual to provide complete documentation. If unable to, complete a 3-dose Hepatitis B vaccine series.
Which high‑risk group may need Meningococcal ACWY & MenB vaccination and boosters; are MenB brands interchangeable?
Microbiologists/others routinely exposed; MenB brands are not interchangeable.
For non‑employee groups requesting immunization services, what should OHS nursing do?
Refer to the Non‑Employee Access Program contacts; do not order/coordinate services unless within OHS scope.
What if your POPA received MMR dose 1 two days prior to submitting their POPA documents? What’s the preferred next step?
Tee up 2nd MMR for 28 days later. They would not qualify for titers at this time.
Employee states they are trying to get pregnant, but a Varicella vaccine is required for employment. What is your next step?
Complete the protocol indicating they do not qualify for vaccination. Review with a provider for next steps.
If an employee presents with record of four or more Hepatitis B doses given in adolescence, and their antibody test is negative, what is your next step?
Consult the provider of the day to discuss if the “extra” doses would be considered booster doses vs lost doses.
o Lost dose – extra doses given to circumstances where they were accidentally given a dose due to lost records, provider change, unknow vaccination status etc.
Does OHS provide minors with vaccines?
Hepatitis B only. All other vaccines must be obtained through the employee/volunteers Primary Care Provider.
