How often must a monthly summary be completed & what is included in a monthly summary?

Monthly. The summary must include how the person did on the goal, if they maintained/progressed/regressed, state success rate and comparison to prior month, address any barriers or concerns, give specific examples of how the person participated in the goal, etc.

Yes! Once the IPOP is signed by all parties, includes all current attachments, and the Staff Action Plan is approved in myEvolv, you can send the packet in!

Yes. The IPOP needs to reflect the change. Even if it is temporary.

The person supported asked for a change, change in health status, a new goal, relocation, changes in providers, significant changes in the safeguards, when the Life Plan is ineffective, adding/removing waiver services, changes that impact billing (effective period start dates, etc.)
