When recouping a claim in FULL (all claim lines are recouped and not one line is left paid), whether money is recouped or the recoupment is done on a $0 paid claim, you will need to apply explain code “2A” to the line, among the other standard explain codes used (26, etc). Used when we intend to void a claim due to a new claim submission.
b. We use code “20” when adjusting a line/claim, where we are increasing an allowed amount or adjusting for COB where no additional money is paid out, but claim is still in a paid status (Claims paid at $0). Explain code 20 should not be used on a line where we are performing a recoupment.
