What should the ticket include?

False: We want to send the information as soon as possible. As soon as you get off of a call you should email over all pertinent information to your supervisors AZDHS.gov email. This helps with time sensitive items like test kits being sent out.

How do we disposition if a case refuses the survey before capturing symptoms?

DO NOT SUBMIT SURVEY - only disposition as Refused unless another disposition is more appropriate (i.e. Already contacted by public health, etc.)

If a case states that they traveled, please document the dates of travel and where they traveled to in the case notes. If they offer any other details, please also include these in the case notes.

What should we do if a Case questions or has conflicting specimen collection date or lab results.

We would want to try to conduct the interview to err on the side of caution. However, if a case is adamant about a negative test or they actually tested for antibodies (igG), we would want to send a ticket with as much detail as possible, including lab name and date. Please also let the case know that they can contact their lab to double check results if they are concerned.

How do we disposition a call if the case refuses the survey at any point after capturing symptoms. This is if they refuse the survey entirely including a DCI.

What do we do if a case refused to continue a phone interview at any point after capturing symptoms but is willing to do a DCI?

If the case refuses to continue with the phone interview with a CI but would like a DCI: DO NOT SUBMIT SURVEY - close out the survey and open the disposition link and select "Email Contact Attempt" and follow the prompts to send the DCI link. Kindly let the case know that unfortunately the survey does not save and it would be starting over from the beginning, however we would be happy to help with any questions they may come across while doing the DCI.

True or False: It is best practice to read the full list of examples for HH contacts/ non HH contacts.

We do not need to read the entire list of who is a HH or non HH contact after already confirming how many people live in the house. This is more of a reference for the CI.

If you are having issues with any Qualtrics survey links, what steps should you take first? What should you do if these steps do not work?

Issues with the survey links (First check the use of incognito mode and clearing the cache and cookies, trouble shoot with CN. If this does not work, a support ticket should be sent along with screen shots of the issue/error in the survey.

If the call disconnects and you call back and cannot reach case - if survey has not gone over contact tracing portion yet (collecting non household contacts), how should we disposition?

DO NOT SUBMIT SURVEY - close out survey and use the disposition survey link to disposition as "does not have time to talk"; Note: the case will need to start from the beginning with the next investigator that calls.

True or False: If we get a case who states that they are a part of Tohono O'Odham Nation, we should end the call and disposition as lives on tribal lands. We should then send a ticket so that the tribe can be directed to the appropriate area to complete the survey.

FALSE: ADHS CICT continues to provide assistance to Tohono O'Odham Nation Health Care with their case investigation. These cases are incorporated in the regular workflow of calls. Investigators should be aware that the survey allows investigation to continue on these residents.

No he is not because CDC recommends a 2nd booster of either Pfizer-BioNTech or Moderna COVID-19 vaccine for adults ages 50 years and older at least 4 months after the 1st booster.

They would be Up to Date: Immediately after getting all boosters recommended for you.

What happens when you call a case and they tell you that you have the wrong number but they are the sister of the person you are calling for? If we are able to get the phone number should we send a ticket to AZ to update this number?

If you get a new phone number always try to call it first to contact the case. If the interview was not completed because you had to leave a VM or unable to leave a VM then a ticket IS needed to update.

Otherwise, if the survey was completed or they refused, please include this information in the notes section of the survey (for a completed) and no ticket is necessary.

What PHI identifiers can be used for a CT survey?

If we only have a name and a phone number, this is enough for verification for the contact to confirm their name with spelling. However, it is in the protocol to ask for their DOB (or age if they refuse DOB) and document this in the appropriate section of the survey.
