When the primary member requests to be removed, a policy split is required. How is this requested?

Complete a processing request with the following information.

Refer to Submitting a Processing Request.

  1. Request Effective Date: Date of the request
  2. Request Type: Referred Endorsements
  3. Request Message: Always include the following:
    • Remove primary, PHID, Primary member's full name, Requestor, Full name of member to be removed, Removal date, Product(s) the primary is being removed from, Reason for removing member

      • If applicable to the reason for removing, include the following:

        • Date the member became Active Duty eligible or Date of the member turned 65 or Date of death.

    • Send To File Room: Do not select

    • Note the policy.
