LOTO (Lock Out Tag Out)
A device used to prevent the unauthorized access or startup of equipment currently being worked on.
What is a Lock?
Riveting, welding, flame cutting or other fire or spark-producing operation.
What is Hot Work?
Part of the fall arrest system that the worker wears on their body.
What is a Safety Harness?
A device used especially during a turnover to prevent the worker from being crushed.
What is a Seatbelt?
A device placed in front of a trailer wheel to prevent trailer from moving or rolling away.
What is a Wheel Chock?
Must be posted with Locked out equipment to identify the employee and purpose for working on that equipment.
What is a Tag?
Required when performing Hot work outside of a designated Hot work area.
What is a Hot Work Permit?
This connects the anchor point to the rest of the fall arrest system.
What is a Lanyard?
An employee permitted by NHUB Management to operate a PIV.
What is a Certified Forklift Operator.
Connect this device to the brake line on a trailer to prevent someone from pulling trailer out of dock.
What is a Glad Hand Lock?
Employees who perform the servicing, maintenance, and set-up of equipment or machinery and those who apply the locks/tags to this equipment.
What is an Authorized LOTO Employee?
Observing an area where Hot Work has been performed for a designated amount of time to ensure a fire does not start.
What is a Fire Watch?
A Process used to check each part of a fall arrest system to ensure it is safe to use
What is a Fall Protection System Inspection?
An employee that trains forklift operators to safely operate equipment.
A Certified Forklift Trainer.
This device rests over the gap between the dock and trailer to allow forklifts and other equipment to drive in and out.
What is a Dock Plate?
Employees whose work operations are in the area where LOTO is being administered, but are not affected by lockout/tagout activities.
What is an Other LOTO employee?
A substance that can catch fire and burn easily such as dust or paper.
What is a Combustible?
Metal hook device that connects the tie-off point to the fall arrest system.
What is a D-ring?
Info posted on forklift displaying the weight and load capacities of equipment.
What is a Data Plate?
An electronic device operated by the dock worker inside to prevent a trailer from pulling out of the dock. (Hint, BWD)
What is a Dock Lock System?
Employees who operate or use the equipment or machines, which are locked/tagged when serviced, maintained or set up.
What is an Affected LOTO employee?
The total amount of hours an area must be watched after Hot Work is performed to ensure a fire does not start or is put out if it does.
What is a 4 Hour Fire Watch?
Must hold up to 5,000lbs or have a safety factor of at least twice the impact of a worker falling from 6 feet.
What is an Anchorage Point?
The Forklift is considered to be stable when the center of gravity is inside this area.
What is the Stability Triangle?
Must be closed if a trailer is not fully secured following our procedures.
