If a plan is made as a less restrictive alternative that includes any collaterals as a part of the follow up, CRS staff must complete these for each collateral and have client sign. CANNOT BE VERBAL
What are Releases of Information?
CRS staff must complete this form with client if: they are 12 and over, client is not being detained, contact was face to face, one has not been completed within the past 12 months, client is not being referred for an intake at FBH

When an individual is seen for crisis intervention, CRS staff must have the individual sign this.
What is Consent for Treatment?
If DCR is evaluation for an ITA, they must provide the individual with this and document it was provided in a progress note if individual refuses to sign for receipt.

In RCW 71.34, the definition of mental health disorder is the same as RCW 71.05 except this sentence.
