The “Panic Value” will be reported preferably to the patient’s physician, but if the physician is not available, the nurse will document the abnormal test result and will immediately attempt to contact the patient’s physician. If the patient’s physician cannot be reached in a timely fashion, the nurse will notify the appropriate Service Chief or designee (e.g. Surgery, Medicine, etc.) to obtain emergency orders. The Testing Personnel will note on the laboratory report who was notified, the time and date, and the Testing Personnel who gave the report using the Critical Test Result Verification Template

Shower rails, pipes radiators, bedsteads, window, door frames, ceiling fittings, wall fixtures, soap and paper towel dispensers, plastic bags, electrical outlets, chair that's not weighted, unlocked carts (anything someone can use to hurt themselves)

What are the steps to follow if a Suicidal Veteran presents in person?

  1. Remain with the Veteran
  2. Alert the clinic manager
    • Clinic Manager will notify VA Police and will ensure that a provider is alerted to see the Veteran. The provider may consult a mental health provider by phone as necessary but it is expected that any SLVHCS provider should be able to assess for suicide risk.
    • Remain with the Veteran until a hand-off is made to the provider.
    • The provider will assess for risk and make a disposition as appropriate.

  • Critical devices, such as surgical forceps, come in contact with blood or normally sterile tissue.
  • Semi-critical devices, such as endoscopes, come in contact with mucus membranes.
  • Non-critical devices, such as stethoscopes, come in contact with unbroken skin.

Ligature points include shower rails, coat hooks, pipes, and radiators, bedsteads, window and door frames, ceiling fittings, handles, hinges and closures.
