 Protected Health Information (PHI) is being discussed in front of an individual who is not authorized to know the information. Staff should discuss PHI only with those authorized and needing to know the information or it should be de-identified. Potentially not using names could de-identify the discussion but if there are other identifying variables this might not be the case.
