When we document, what do we call the people we work with?
BY THEIR NAME: historically, we would refer to people as "client or consumer". As clinicians, we never refer to people as "patients'.
What are some of the goals of the services that we provide?
Improve symptoms or maintain them at baseline, develop age-appropriate functioning, improve functional status, prevent new morbidities.
What does GT stand for?
Golden Thread- the consistent presentation of relevant documentation
What the heck is a widget?
Widget is a feature in Avatar that pushes forward information to help you track and find things (example: your Forms & Data widget will house all of the forms you may need to complete)
True or False: Penguins can be found at the North Pole.
False: penguins can be found in Antarctica and even in parts of South America (and other regions). However, you will not find them in the North.
What does "PHI" stand for?
Protected Health Information: this includes demographic information, medical history, or other information that could identify an individual.

How does the Golden Thread work?
•Intake Assessment: identifies an appropriate clinical The Golden Thread begins with an intake assessment that clearly identifies an appropriate clinical problem and corresponding diagnosis.
•Treatment Plan: reflect a clear series of goals for helping the client through the identified problem. Each goal should have specific interventions prescribed that reflect best practices and evidenced-based treatments to help guide the client along the path to recovery.
•Progress Notes: demonstrate that the services you deliver match what was prescribed in the treatment plan. Each note should lead into the next, creating a comprehensive story of the client's progress through treatment.
What is an episode?
A defined set of services that an individual receives with a distinct start and end date. An individual can have multiple episodes open at one time (e.g. a person can be enrolled in a clinical, psychiatry, and residential episode simultaneously)
