Please list all the steps for requesting PTO.
Request for time off must be made 2-weeks prior to the requested dates.
A “Request for Time Off” form must be filled out and submitted to Tara or Cortnee.

If the request is approved upon review, staff must submit for PTO on UltiPro.
After administration has approved the request in Ultipro, an outlook calendar notification will be sent to the rest of the team, so that everyone is aware that staff are out on that day.
