Miyad lepeh is that once the mixture is in the stage where it is between your hand and your mouth, you are allowed to remove the pesolet. However, the Chazon ish doesn't like this and says that you should hold onto the pesolet while pulling away from the ochel, so it is ochel mitoch pesolet.

The taste (ta'am) of the food is what makes the distinction of ochel and pesolet.

we say you don't need to eat it and you still won't violate borer since initially you thought to eat it miyad and something out of your control happened so you fulfilled miyad in the moment of taking.

By dumping out the salad, dispersing it and removing the pit.

Rabeinu chananel says that you can do it at any time DURING the meal
rav Mordechai says that you have one hour beforehand that you could prepare
Rama says you can only do it as you are sitting down and during the meal.
Bottom line ashkenaz/sefaredi: before the meal you can take the amount of time that it usually takes to prepare the meal as long as you don't have a significant activity in between the preparation and the meal and the meal itself.
If there is a thick film over a bowl of soup, which I could easily remove with my hand, can I remove it? can I remove it with a spoon?
You can remove it since it is blocking the ochel so it is siluk, and you could remove it with a spoon since it is not doing a better job then you at taking the film off since you could just as easily remove it with your hands and it is therefore not a specialized utensil.
