In the monthly paperwork, these are the most frequent missing items.
What are agendas? (admin, A/C, & team)
They are the ONLY person allowed to edit a student profile; including adds, drops, name changes, etc
Who is the ICT? (Lainey)
This event takes place within 15 minutes of the dismissal bell.
What is attendance?
The ONLY thing a Teaching Fellow is allowed to use to clock in or out.
What is the CTFF computer/iPad?
Fun, intentional, and organized; this component of program needs to have constant supervision and is never used as an incentive or consequence.
What is Rec?
Those who audit theirs daily have drastically lower corrections at the end of the month. This should sit in a central location and be a point of contact between staff and families.
What is the student sign out binder?
These are required for daily use, turned in monthly, and should be posted in your site folder and kept updated in the event you are absent.
What are head counts/rosters?
This is a written documentation for parents, site, and leadership after a meeting or conversation with the site facilitator, student, and parent.
What is a behavior notice?
The day of the week CitySpan attendance is due.
What is Monday?
When these monthly events take place, remember to post signs for families picking up.
What are safety drills? (lockdown, shelter in place, fire)
One bears your portrait, the other is a wonderful burgundy. Both grant you permission to be on campus.
What are ID badge and Bridges shirt?
The goal of this component is to provide a new healthy experience for students, sometimes it's cultural or something they've never had before.
What is nutrition?
A student with one of these in their file has permission to sign themselves out under specific conditions and must leave campus immediately.
What is a walking permission form?
The place to enter your minimum days, look ahead for meetings, and respond to events.
What is the Outlook Calendar?
This form is used for anyone from an outside agency that has been verified by leadership and has a reason to be in program, including aides, nurses, guests, and those seeking extra credits or hours.
What is the volunteer log?
These items are organized, easy to find quickly, and correspond to an activity.
What are back-ups?
Not just a mini computer, this piece of equipment needs to be available for the entirety of the Site Facilitator's shift. It is the main source of communication between the families and staff, and between the regional office and site.
What is the site phone?
This daily greeting from leadership to the Site Facilitator gives them the information needed to plan their day. If there are changes, they should notify the regional office immediately.
What is the daily staffing photo?
This is the second, complimentary, component when planning and executing nutrition. It's used as an educational tool and explains how and why the nutrition is beneficial.
What is the nutrition lesson?
You give this out to families if there is a reason a student will not be attending Bridges every day, or when they are attending a parallel program on campus.
What is a special activities form?
Hours you should be at program, accessible, and available.
What is 10:00am-6:30pm?
Site Facilitator and program assistants (if needed), rec aides, and sub sign outs. These are the only ones that should be scanned over.
What are time sheets?
The specific place were you can print extra forms or documents, like sign out binder cover sheets, staff emergency cards, or blank CQI pages.
What are the site folders?
Use Q or check with office staff to verify or confirm absence, call family, call emergency contacts, call leadership. These are the steps you take when________.
What is, "when a student does not check in"?
Calling, texting, and/or emailing ALL leadership with as much advance notice as possible, making sure you have acknowledgment from someone.
What is reporting an absence?
Once made, this helps your staff and sub should you be absent. Additionally, any items purchased should match this when reviewed monthly.
What are activity plans?
A line between a staff member and students or family that keeps things professional.
What is a boundary? (are boundaries)
These items are not to be purchased during supply pick-up. Donations are a better option.
What are incentives?
This tells you in what order your paperwork should be, it even tells you what to group together.

General location of where to find extra forms, also where you put activity plans and back ups.
What is the W drive?
Blind folding students, ouija boards, axe throwing, original lawn darts, spin the bottle, Red rover, piƱatas, Twister, tug of war, tag, dodgeball, three legged race, milk chugging, potato sack racing, etc
What are banned items or activities?
This is how all staff should conduct themselves at work, including clothing, communication with leadership, families and students, and use of district devices.
What is professionalism?
This block of time is for required reading, educational games, grade checks, and work that needs to be completed.
What is homework/academic time?
If one of these occurs, you or a staff member should notify leadership within 5 minutes.
What is a late pick-up?
Ongoing self audits to make sure your site aligns with the established standards.
What is CQI?
This report is run, checked and compared line by line, and all copies turned in monthly.
What is the 'students by number of absences' report?
This is how often, at the minimum, you should be reporting to leadership when students are not attending program (absent every day).
What is monthly?
Once this form is filled out it is sent to Leadership via email, the original turned in the following day, and no other copies are to be kept or distributed including on site student files or sent to teachers/parents/admin.
What is an incident report?
Used daily, this is a record of hours worked for all staff.
What is the staff sign out binder?
______ is when the students are learning in a fun and engaging way, often times not even realizing they're learning.
What is Enrichment?
A set of clearly defined expectations approved by leadership that is consistently upheld with ongoing communication with families.
What is a behavior system?
The applications we do not accept.
What is none? We accept ALL applications!
