Screening Eligibility

A family who is expecting ( any stage in pregnancy) or has an infant under 3 months of age. 

Anyone can refer a family to us. You can do so via phone, or email.

Healthy Families assessment tool

Yes, we use the parent survey with the family's consent. This participation is voluntary. . This tool assess for risks/areas of need to better help us understand how we can help the family. If the family scores 25 or above in the 10 domains then we can offer Home visiting services. If they do not score 25 or more then we will make sure to offer referrals to address the needs that they identified.

Families who live in transitional housing

Absolutely, we serve families who are living in family shelters.

Depression Screening

We use the PHQ-9 to assess for depression. We connect families to mental health providers if needed.

Social gathering

Yes, we facilitate events  for participants to help build social connection and reduce social isolation. Currently, groups are virtual.

Home visiting services

Home visiting is voluntary. We offer them bi-weekly home visiting services up until their third trimester. Visits then are offered weekly after their third trimester up into the child is approximately 6 months old. After this, we review goals with the families and offer different levels of frequency starting with bi weekly, monthly, and then quarterly as the child is transitioning to graduation.

Languages spoken

Yes, Currently we have spanish speaking staff. We are working on hiring staff who speak other languages such as french, creole,Bengali, etc. Please refer any candidates our way.

Developmental screenings

We use the ASQ and ASQ-SE at different intervals in order to assess for any potential area of need/concern. We refer to Early Intervention with Parent's understanding and consent.

Breastfeeding

We support participants with breastfeeding. Some of our programs have staff trained in CLC, Doula, and we conduct breast feeling support groups. We also have community partners that we can refer them to.

Healthy Families graduation

They graduate when the child is between 3-5 years old. The family can continue to participate until age 5.

Covid 19

We are conducting virtual home visits as of now. We have been able to conduct some visits outside when the weather permits following social distancing practices.

We screen for domestic violence using the HITS assessment.

Fatherhood

We engage fathers by facilitating groups, and making sure that they are engaged in home visiting. We  facilitate workshops and father specific groups.

Family members

Yes, we focus on the family as a whole. We involve dads, partners. grand parents, and anyone else who is part of the family.

Health Insurance requirement

They do not need health insurance to join our program.  We connect them to health insurance if they do not have any.

Parenting Classes

Some of our programs offer a 12 week psycho educational group called parenting journey for free. We also offer hand on parenting support in the home by using our evidence based curriculum.

Yes, we do. As long as there is no order of protection between the child/parent we work with both parents during home visits.

Healthy Families is absolutely free.

Parent child attachment

We use the Cheers Check In tool to assess for areas in which we can support the family with bonding and relating to their child in the best way that they can. Families are already doing such an amazing job and can enhance their interactions with this tool.

Goods

We may be able to provide goods when there is supply such as diapers and basic care items. We however do not have supplies year round. we refer our participants to community partners who can offer these and use their guidelines for eligibility.
