WS presents the ED complaining of a low grade fever, night sweats and increasing weakness over the past week. Blood cultures are positive for MRSA, and a TEE demonstrates a vegetation on the aortic valve. WS has NKDA. what is appropriate empiric treatment per IDSA guidelines
