What does PACE stand for?

We often say PACE is Complete healthcare and more for seniors living at home - it tells a bit more about PACE.

How do you refer to PACE?

It is easy, all you have to do is give the referral's name and number to a navigator. Anyone can refer to PACE!  Do you know someone that could benefit from PACE?

Many of our referrals come from word of mouth from our employees and our participants!

Does PACE accept Long Term Care insurance?

No, we do not accept Long Term Care insurance.

What does L.T.C. stand for?

Long Term Care 

To be eligible for PACE you need to qualify for nursing facility level of care/LTC in Virginia - meaning you need to have assistance or for some of your activities of daily living.

Do you have to have Medicaid to enroll in PACE?

No, some of our participants are private pay.  

Also, we  help many of our potential participants apply for Long Term Care Medicaid, it is often part of the enrollment process.

What does D.S.S. stand for?

Department of Social Services 

We work with the Department of Social Services to assess our potential participants to confirm that they meet nursing home level of care in Virginia.

How long does it take to apply for Medicaid?
The Medicaid application with Department of Social Services can take up to 45 days.  We often have to do a lot of work with the potential participant/family to help them find/request financial information for the application - bank statements, life insurance statements, Social Security

If a participant lives in Nelson County can they enroll in Centra PACE?

No, Nelson County is outside our service area.  Nelson County is served by InnovAge in Charlottesville, VA.

How long does it take from the first contact with a possible referral until someone enrolls in PACE?

It varies!  It can be about week (rare) or it can be few years before they are "ready" and enroll.

What does the abbreviation MCD stand for?

Medicaid 

Did you know that most of our referrals don't have Long Term Medicaid and we help them apply for Long Term Care MCD? Also, Long Term Care MCD is easier to qualify for than Community MCD because you can own your own home and a car.

Why do some potential PACE participants go through some or all of the intake and enrollment process and then not enroll in PACE?

Not ready for a change -- Family member wants to be a paid caregiver -- Unable to pay the copay -- Need more time to make decision -- PACE is too good to be true -- PACE is not the right care model for them -- Don't think they need the help -- Don't want so many folks involved in their business -- Worried that next step is a nursing home -- They just say no -- Medicaid not approved -- Spouse/family doesn't support PACE enrollment -- They are higher functioning than the participants they see in the center --  They want to stay home...

What is the primary reason we don't approve a potential participant for PACE?

If we don't approve a participant for PACE it is usually because of a safety issue.

What are some benefits that PACE provides our participants?

So many benefits! Some are...

Supports independence for seniors -- team approach to care -- meet participants where they are -- 24 x 7 on call provider services -- care for the whole person -- more than healthcare, includes wrap around services to help seniors live at home -- supports caregivers as well as participants -- reduced hospitalizations and readmissions -- individual care plan and reassessment of needs --  in-home care and virtual programs -- PACE is able to think outside the box to care for our participants...

What does U.A.I. stand for?

Uniform Assessment Instrument (U.A.I)

A UAI can be done by Department of Social Services and the Health Department in their resident county. The UAI assesses whether or not they will qualify for PACE by meeting nursing home level of care requirements. A UAI can also be done by Case Management in a hospital. It is quicker to get the UAI done by the hospital because the UAI by DSS and Health Department can take 2 weeks or more. Since the pandemic the UAIs done outside the hospital have taken longer.

What are some of the steps for the intake/enrollment process of PACE?
