Form is on SharePoint and completed monthly.  Some of the areas to be reviewed to ensure in proper working order: appliances, blinds/shades/curtains, carpet flooring, doors and door locks, check fire extinguishers and smoke/co detectors, plumbing, lighting, emergency phone list and evacuation plans. Remember to check for issues outside as well! 

Document any conversation you have had in order to get items resolved as well!

Name 5 reasons that an IR would need to be filed. What is included when submitting an IR.

ANE, Bruises or lacerations larger than 3 inches or from an unknown reason, falls, medication errors (missed medication, wrong dose, wrong route), ER visits, issues at the site that cause it to be unlivable (power outage, fire or tornado (storm) damage), peer to peer aggression with significant injury, death, elopement, significant injury, criminal activity, aversive technique, restrictive measures, or use of a PRN.

Include name, person supervising, diagnosis, age, living arrangement, comprehensive description of what happened, activities that occurred prior to the incident, injuries sustained, immediate actions that happened and an planned actions, first name and last initial or all those involved.  If ANE or Death, you must report incident to CPS/APS.
