What is EXTERNAL case management and when do you get it?

External case management is an outside case management group that will be assigned to you and continue to follow with you as you discharge and live in the community. You will receive this between the 6-10 month timeframe at Casa Pacifica.

Examples: Downtown IMPACT, North Coastal ACT, Telecare La Luz/Vida/Tesoro, Catalyst.
