What are the Client Service Plan/CSP and the Service Plan Review/SPR?
This service is provided to our clients to assist them with medical, social, educational, and other services. The team develops case plans to coordinate evaluation, treatment, housing, and care, tailored to each individual’s needs and preferences.

This is a tool used to assess a client’s level of suicide risk to assist the clinical team in developing appropriate interventions.

The client was as needed case management client, but recently experienced grief and substance abuse, which caused the client to go to Detox. The SPR/CSP now needs to include.
What is High Risk and Substance Abuse goals?
This service provided by Apalachee assist clients with learning how to Process trauma, better manage their symptoms, and responses to situations, gain better control over their day-to-day mood and overall functioning, and learn how to approach situations in a positive way.
What is therapy services?
This form must be completed for all clients of Apalachee to inform them of their rights and responsibilities, program rules, procedures and activities, services, and treatment planning.
What is the Acknowledgement of Orientation?
This letter is sent to clients after 90 days of no service and after several attempts to reach the client either by phone or mail. It provides clients 10 days to respond.
What is the Notification to Close Letter.
This tool collects the history of psychiatric, psychological concerns, as well as social factors that affect the client and his/her relationships in order to fully understand the current status of the client’s needs.
What is the Comprehensive assessment?
The client struggles to become motivated to work/look for a job. He relies on his social security application to get approved. This resource could be added to the client's CSP/SPR as an objective.
What is Goodwill/Vocational Rehab/Career Source?
These are the clinicians who assess, diagnose and treat symptoms. They also monitor the non-psychiatric medications and work closely with the client’s primary care physician.
Who are the Psychiatrists and the Advanced Practice Nurses(APRN)?
Due to HIPPA, this form must be completed and in all clients' charts when completing psychiatric appointments using technology.
What is the Consent to participate in telemedicine Services form?
This outcome measure is completed for all clients upon discharge and is a state-required performance measure consisting of 18 problem severity ratings.
What are FARS?
This tool is used in a time of crisis. It is set up as a series of steps (1-6), and a client should start at Step 1 and work his/her way through the tool during a crisis.
What is the Personal Safety Plan?
The client's CSP/SPR will expire in 15 days. It has been developed. The APRN has a number of days to sign before the CSP/SPR goes into effect.
What is 10 days?
This service is designed to provide comprehensive and early intervention to individuals who have experienced a first episode of psychosis. It provides services to clients between the ages of 15 and 35 who has a diagnosis of schizophrenia, schizoaffective disorder or schizophreniform disorder

This form is required as appropriate to document a client's and/or guardian's informed consent for the use of psychiatric medications in their treatment of services.

This form serves as a tool to identify active care issues, issues that have been resolved, and those that have been deferred for a later time. It is the driving force of the CSP/SPR, and the information is gathered from the assessments completed.

This service offered by Apalachee is not focused on diagnosis, disabilities, or deficits, case management or other clinical services. a replacement for other services, and is not the only tool for recovery.
What is Peer Support/Peer Specialist.
This form lists key information that the client needs to be informed of and should be discussed at the time of intake/initial appointment with the CM/SC/ICC staff.
