What is Amida Care pharmacy?


The Amida Care pharmacy department is a great asset to know and use to possibly review member's regimens. Call AC pharmacy when necessary for help with follow-up with a community pharmacy member may be having issues with. Possibly schedule a phone conference with member and pharmacist to review regimen if they have concerns about their medications. sometimes reaching their providers may be hard, and pharmacist have medication knowledge they may be in search of.

What is Advance Care Management?


Reminder that units are measured in 15 min increments. There should not be more than 10 units for your notes. The only time use use 1 unit is if the interaction was unsuccessful. When documenting ACM units >5 please substantiate the work with your member in documentation (health education, case conference, hospital visit, etc. )

Care Alert - Psychosocial Reassessment Status: Past Due 

What is what prompts me to do a psychosocial assessment?

Amida Care is required to do psychosocial assessments of member every 6 months. Everyone at Amida Care should be completing these assessments, and RICU can help with this too. The assessment may even be things you're already aware of and can easily ask member. The information you obtain of things and barriers they are facing may even be beneficial in your work with the member. Janelle did a training for how we should be doing. Rafael is our internal expert on how to complete.

48 hour follow-up

What is the time frame for following up with a referral?


When assigned a member you have 48 hours to initiate outreach, unless asked to reach out someone in a shorter timeframe for a hospitalization example. Member's referred to RICU are in need of our support. Our RICU policy clearly states we should follow up with referrals made to us within 48 hours.

VL suppression is attained when an individual with HIV has a viral load of <200 copies. Some labs may even show <50.

What is the amount of time I have to enter a note?


Notes should be written as soon as possible, but no later than 48 hours. Your notes communicate to others at AC what you're doing with a member. It helps collaborate in a timely fashion. It also helps with reporting.

Claims and Authorizatons

Where I look for last filled medications or last PCP visit?

Claims for Rx and appts are important information you can use to educate your members and provide health promotion. It can help mold your next conversation if your member has missed a prescription fill or they haven't' seen their PCP or specialist in a long time.

Tuesday

What is our mandatory in office day?


We only require folks to be in the office on Tuesday, please attend, and avoid scheduling appts when possible on this day. Folks who can not attend Tuesdays will be asked to work with their supervisor to be in the office for their administrative day on another day.  You miss a lot of important information and support from your peers when you're not in the office with your team. When possible if you need to be out for an appt, please try to schedule for alternative days. It will be your responsibility to obtain the missed info from your supervisor on your next day in office.

What is the # for IT?


This number should be saved on your personal phone, If your work phone gets locked out, they can unlock but you can't call if your phone is locked. If you have issues with your hardware please resolve immediately. We need our laptops and phones to conduct our work. You should not wait until next time in office to address. JIRA tickets must be submitted for items IT helps us with. If you need help with this, ask your supervisor.

Outreach Checklist

What is what I use to prepare myself for a visit?

RICU checklist helps to prepare and educate you on your member prior to going to visit them or calling them. It is a tool we use to research their salesforce profile and know what items need to be emphasized to member and what areas related to members social barriers and health need attention and support.

What is how many attempts I should make to find someone assigned to me?

RICU policy states we outreach folks to all addresses listed 2x and make phone calls to all #s 2X. these outreaches when a member is first referred should transpire within 30 days of referral, with the first outreach happening within 48 hours.

CD4 and Viral Load

What is the labs commonly used to monitor HIV?

CD4 or tcell count measures the strength of the immune system by counting white blood cells (normal: 500–1,500 cells/mm³). Viral Load (VL) measures the amount of HIV in a blood

What I need to do when I need help with ADP (clock in or clock out issue, I am out sick)?


When you call out, texts although great, are sometimes overlooked and not readily available for supervisors when fixing ADP at the end of a pay period. Any item that may need fixing in ADP should be emailed to supervisor within that pay period. Supervisors are required to write notes for any edit we make. Your email facilitates the comments we add to ADP, and is documented in a place for your supervisor and Director to use.

Resistance

What can happen if you skip your HIV medications frequently?

HIV drug resistance occurs when the virus mutates and continues to replicate causing treatment to fail and a person becoming resistant to a medication. Adherence to regimen is important to avoid this. Drug resistance testing can be done to confirm, members can talk to their PCPs about this.

Care Plan Notes

What I should read to see what others are doing with a member, current and past?

All health services notes are care plan notes. Other health services members may have documented recent notes which may be important for your next visit with member. Becoming familiarize with what others may be discussing with your members can help you move your members forward.  OR their note may even be related to a move and a new number.

ADP 2 weeks prior

What I need to do to schedule PTO?

We are a large team and PTO should be requested in a timely fashion. Entering request in ADP 2 weeks prior allows us time to approve, and ensure their is team coverage.

Adherence

What's following a prescribed treatment regimen?

Adherence means to follow a plan, a following directions. Related to the work we do, a member can be adherent to their medication which means they follow their regimen as prescribed by their provider. Related to their appointments, adherence means the member adheres to their appointments and sees their prescriber as scheduled. Both are important.
