•“The Adolescent Behavioralist” is an interdisciplinary solution to improving the health of the young adolescent population in Chelsea, Massachusetts. •“Closing the loop: An IT intervention to improve adherence” is designed to address poor adherence to prescription medications. •“An Integrated Tai Chi Intervention for Fall Prevention and Wellness Promotion among Elderly Patients at Bowdoin Street Health Center” is designed to implement a Tai Chi-based learning curriculum for fall prevention and wellness promotion in the community health center setting. •“Refer Smarter” is a project designed to create value and efficiency by using an asynchronous remote opinion system to connect primary care with specialty knowledge.
