How many people are allowed to speak at a time?

One! One person speaks at a time so that everyone can listen to each other without interruptions. With less interruptions, there's less confusion and frustration. And you can end your meeting sooner!

Why are program staff at my meeting?

Program staff attend CFT meetings to inform your team how you have been doing in the cottage, how you are getting along with others, how you are taking care of yourself and handling your responsibilities, etc. They are also there to help advocate for you in having your needs met.

What is something you can do to help yourself remain patient for the rest of the meeting?

Use a self-regulation skill like deep breathing, doodling, using fidget toys, writing, etc. You can also ask the facilitator for a short break.

Are action steps bad to have?

No, they are formulated goals your team comes up with for additional services, skills to develop, or for youth to continue with their progress

What does Program Staff do?

Provides safety and around-the-clock supervision, provides an atmosphere that is supportive of the needs of the students; monitors and documents student behaviors and activities; participates and assists with educational, social, athletic and recreational activities; works in collaboration with other professional staff to establish and/or meet the goals of each student’s treatment plan.

Is shaming and blaming allowed?

Shaming and blaming are not allowed because doing so does not resolve anything. One of the purposes of CFT meetings is to find solutions that build on your strength.

What does your Therapist provide?


Provides a Care Plan with goals related to youth's mental health diagnosis. Will provide individual therapy to help stabilize emotions to help youth transition to a lower level of care.

Name five youth action steps. You can think back to your last CFT meeting for ideas.

1. Follow program rules and expectations (general action step - this covers going to school daily, attending appointments and groups, etc.).

2. Work with a specific provider to work towards a specific goal (i.e. Meet Kim 1x a week to work towards transitional goals).

3. Communicate with specific person/people to have needs met (i.e. Talk with teacher to develop a plan for when I need a break).

4. Work towards a goal to earn an incentive. (i.e. Go to school 5 days a week to earn $5 bonus allowance).

5. Any goal that ultimately helps you with transitioning to a lower level of care (i.e. THP, TFCO, reunification, etc.).

Who is our Transitional Care Coordinator and what do they do?


Kim O. Assists youth with developing independent living skills and provides case management services for youth to transition back into the community independently. Also facilitates CFT meetings.

Who is our Mental Health Behavioral Specialist and what do they do?

Michelle S. She provides mental health rehabilitation services (coping skills) for youth in our program in order to ensure stability in the home. Helps with successful transition back to the community.

What does CFT stand for and who is allowed in CFT meetings?

Children and Family Team Meeting

Anyone who youth would like in their meetings, as long as there is a release of information signed. If there is a no contact order or restraining order, that person is not allowed to attend.

What is my CFT?

A group of people that includes you, your family members, trusted adults, caring professionals, and others who work together to reach goals for positive change. The team supports YOU! The team provides support and encouragement to achieve your goals and your family’s goals.

Why do we need a signature page and release of information page?

To acknowledge participation in the CFT meeting and to acknowledge that information exchanged among the CFT is confidential and may not be further disclosed.

Action steps are collaborative and those participating can provide input for action steps in order to help youth transition to a lower level of care.
