Document the case per incident reporting requirements, locate the separated parent or legal guardian, document efforts in case management notes, and notify the child’s attorney of record, LSP, and Child Advocate within three business days.

According to Field Guidance 24, what are the Case Managers responsibilities when it comes to sponsor outreach and communication?

  • makes initial contact with potential sponsor to introduce the program per UC MAP 2.2.2. 
  • Confirm sponsor intent, capture contact information in UC Portal (See Quick Reference Chart: Sponsor Record Entry Schedule and Location), identify additional potential sponsors using the “snowball method,”  
  • identify any potential health conditions the child may have and escalate to the Medical Coordinator within 60 minutes of disclosure, and  
  • explain the role of the Unification Specialist. 
  • Enters sponsor information into UC Case Review (Days 1-5) or UC Case Review (Days 6 and beyond)
  • Emails the Unification Specialist to notify them that a sponsor has been identified and provides the contact information for the sponsor.

If a Category 3 sponsor is denied, who is responsible for informing them, and how?

The Unification Specialist verbally informs the sponsor of the denial and documents it in the UC Portal.
