A patient at this level will only be able to pay attention for a few minutes. May require step by step instructions for an activity. This patient may still have difficulty making sense of things outside of the patient.

The tendency of a patient with a TBI to act without thinking about the consequences which can manifest as blurting out inappropriate comments or running out in the street?

This tool measures a patient's satisfaction or issues that bother a patient with TBI with various aspects of health conditioning, functioning and well being.
What is the Quality of Life After Brain Injury?
A patient that may track with their eyes and show minimal spontaneous movement but does not respond to verbal stimulation.
What is a Vegetative State?
This type of learning is implicit learning, meaning it occurs unconsciously and leads to the automatic performance of skills.
What is procedural learning?
Able to handle multiple tasks simultaneously in all environments but may require periodic breaks. Able to independently think about consequences of decisions or actions but may require more than usual amount of time and/or compensatory strategies to select the appropriate decision or action. Accurately estimates abilities and independently adjusts to task demands

A patient with a TBI may get stuck on an idea or an activity.
What is Perseveration?
This scale measures resistance to distraction, consistence of attention over time and ability to make decisions about deploying one's attention over the course of 2-3 days

A Standardized Neurobehavioral Assessment measure designed for use in patients with disorders of consciousness. This scale is used in the rehabilitation settings.

The ability to maintain a consistent behavioral response during repetitive activity. Also known as vigilance
What is sustained activity?
Begin to recognize family and friends. Follow some simple directions such as “Look at me” or “Squeeze my hand.” Begin to respond inconsistently to simple questions with “yes” and “no” head nods or shakes.
What is Ranchos Los Amigos Level 3: Localized Response?
Sudden and intense and unpredictable mood swings, often involving exaggerated or involuntary crying or laughing that doesn't match the situation or the person's true feelings.
What is Emotional Lability?
It is a 16 item that assess multiple cognitive domains including visual-spatial and executive functions, naming, memory, attention, language, abstraction and orientation. It can be used with patient's with TBI or PD with mild cognitive deficits.
What is Montreal Cognitive Assessment (MoCA)?
The consciousness is severely altered but there are signs of self and environmental awareness. This patient is able to follow simple one-step commands inconsistently, may even demonstrate some verbal response to stimuli.
What is a Minimally Conscious State?
The ability to maintain a behavior or cognitive set in face of distracting or competing situmuli.
What is selective attention?
Patient is able to follow a set schedule, and able to do routine self-care. Patient will still have problems planning and paying attention. Thinking may continue to be inflexible or rigid

A TBI often tries to fill in gaps in memory by making things up.
