What is the protocol book and where is it kept?

The protocol book contains all agency policies and procedures, certificates and training sign offs and is kept in Becky's office.

What is and ISP? How often is it held? Who are the attendees?


Individual support plan conducted for residential and employment services and supplemental funding. Held once a year, includes provider(s), service coordinator, individual and family members.

A: The emergency evacuation plan is kept in each office/activity room and describes the policies and procedures for an evacuation in the event of an emergency.

Where is the information located?


An individual can talk to any staff as we are all mandated reporters, can talk to their human rights officer as well. They can also DPPC (Disabled Persons Protection Commission) 24/7.

How are staff kept up with procedures and policies?

As policies are reviewed and updated, staff are trained on them, as well expected to read through and sign off upon program orientation. Policies may be reviewed at weekly staff meetings or morning meetings and all staff sign off. Staff may also access the protocol book at anytime or find the policies drive on the computer.

Ongoing monthly discussions of human rights one of the month and an annual review training for the participants and staff.

We review day hab goals on a monthly basis at clinical team meeting, discuss weekly at staff meetings, adapt goals/techniques for teaching as abilities change; give an example!

What is the satisfaction survey and who is it given to/when?


A satisfaction survey is used to provide feedback to communicate as systems/programs from participants and caregivers regarding aspect of satisfaction with programming. It is done on a yearly basis.

How are we making sure that people have access to technology and supports what they need?

We acquire new technology as it becomes available (example: iPads),we have trainings by our clinicians (example: communication,signs and symptoms of illness, behavior trainings, etc.) Annual reviews of adaptive/supportive devices and behavior plans, discussions with family/caregivers, etc.

What is the procedure for reporting an incident? What information is included in this form? 

Staff fill out an incident report and submit to Director/ Assistant Director. Internal incident report form includes a summary of the incident with time/date/location of the occurrence, if there is and injury/property damage, if Health Care Coordinator was notified and treatment provided,  and any other steps taken to protect health and safety of the individual.
