This is a new task under Homemaker and Personal care services and involves functional skills training to accomplish ADL's and IADL's to enhance member's independence.
What is acquisition, maintenance, and enhancement of skills?
What is 16 hours a day?
Tasks such as shopping, menu planning, grocery shopping, meal preparation, banking, money management, and appointment management are available under this service under CFC.
What is homemaker?
Newly enrolling members requesting HCBS-SLS day services/supported employment have to complete this assessment following the 100.2 and financial eligibility is confirmed.
What is the (Interim Support Level Assessment) ISLA?
This service is available to members transitioning from institutions or with demonstrated need.
What are home delivered meals?
If a member is newly enrolling into CDASS or IHSS, refer the member to his agency for member orientation.
What is Consumer Direct of Colorado (CDCO)?
A legally responsible person (LRP) can only provide up to this many hours a week of homemaker.
When entering in CFC services into the Bridge, CFC services must have this modifier.
