Little Timmy is standing his first solo watch on Unit 2. The 4th license is on a bathroom break when an electrical fault on 2X04 caused it to lock out. In the confusion of all the alarms and flashing lights, Little Timmy inadvertently initiated a Manual SI. While he and the OS are discussing plant restoration, the fourth license notices an alarm tile lit on 2C20 B 1-9.

What is the most likely cause of this alarm tile and what needs to be done to restore the alarming equipment to service?

D-08 Battery Charger trouble Alarm.

Need to either reset SI OR shut normal feeder to 2A06. Then reset and shut the contactor.

Little Timmy declares that Unit 1 RTA is inoperable. Per TS, how long to we have to restore this breaker to operable status?

IF RTB is operable: 8 hours

IF RTB is not operable: 1 hour (or within the Risk-Informed Completion Time) to restore RTA to operable status

27

Both units are at RTP. What is the Minimum number of qualified bodies required to make up a watch team. (Licensed and Non-Licensed)

2 SRO

4 CO

1 Lead AO

2 EP-EPies

4 AO's (Fire Brigade)

231

The electrical distribution system is in a normal configuration with the exception that 2DY-03 is Out of Service and the Unit 2 white channel is being supplied by DY0C. I&C convinces the Unit 1 OS that they need to do urgent calibration within 1DY-03 and need to take it out of service. The OS directs you to place Unit 1 white channels onto DY0C.

Is this possible? Why or why not?

Physically, yes it is possible. However, administratively we can only have 1 unit on the alternate inverter at a time.

231

Normal and Alternate Power supplies for Unit 2 Charging Pumps: ____?

A: Normal 2B03/ Alt B09

B: Normal 2B03 / Alt 1B03

C: Normal 2B04 / Alt None

231

What will cause CREFS to shift to Mode 5 (automatically)?

Explain what happens to fans in the CREFS systems upon a valid signal to shift to Mode 5, include all applicable time delays if fans start AND fail to start.

A Loss of Power 1Y06 / 2Y06 OR one of the Automatic Initiation Signals

Either train Containment Isolation from either Unit, RE-101 High Rad, RE-235 High Rad

These contacts open to deenergize the arming relays, AUX-1 & 2. – (Trip running Fans)

This causes the following:

    • Deenergizing of 3 Time Delay Drop-Out (TDDO) relays.

        — TDDO-2 is set at 20 seconds for starting W-14B and W-13B2.

        — TDDO-1 is set for 41 seconds to start W-14A and W-13B1 if interlock is met.

o Interlock: If W-14B/W-13B2 does not start it receives a stop signal and W-14A/W-13B1 starts

        — TDDO-1A is set for 50 seconds for Low Flow Alarm on C01.

            • Opens a contact to trip W-14A and B and W-13B1 and 2

231

Unit 2 is in Mode 6 preparing to unload the core. Little Timothea, the sister of Little Timmy, comes into control with chemistry sampling reports for the shift. OS2 is reviewing the SFP boron concentration and notices that boron concentration is 2200ppm. Is this in violation of Tech Specs? If so, what should OS2 direct the watch team to do?

Yes. 3.9.1 Boron Concentration states that the RCS, Refueling canal, and the refueling cavity shall be >/= 2350ppm [B].

Suspend activities that will add positive reactivity - Immediately

Initiate action to restore boron to within normal limit - Immediately

231

Per administrative procedures, what are the work hour restrictions regardless of unit mode?

  • No more than 16 work hours in any 24-hour period
  • No more than 26 work hours in any 48-hour period
  • At least a 10-hour break between successive work periods, or an 8-hour break when a break of less than 10 hours is necessary to accommodate a crew’s scheduled transition between work schedules or shifts
333

You're preparing to take the watch as the 4th license. During your board walkdown, you notice the Diesel Mode selector switches in the following positions:

G01: Auto

G03: Exercise

G04: Local

G02: Exercise

Assuming all other EDG switches are aligned for normal steady state operation, what EDG's will start if both units simultaneously have an SI actuation?

G01, G03, G04 will auto start to ready to load condition

G02 will not start

333

Little Timmy is getting a knowledge checkout on RCP seals, he's really confused about the shutdown seal. Explain to him what this seal is, how it is activated, and what needs to be done on indications that it has activated.

It's a seal that is located between the #1 and #2 seals. It will auto activate at 280F, if actuated the RCP shaft should be stopped to prevent damage to this seal.

Prevents a loss of reactor coolant system (RCS) water inventory should an event occur that causes a loss of all seal cooling

333

PT-478 fails to 1100psig. What happens to Rx power and why? (Assume Little Timmy is on watch and blue screens)

Rx power will rise due to a rise in steam demand from 1MS-2015 popping open.

333

Unit 1 is Mode 1 at RTP, Unit 2 is shutting down for outage and is currently at Tave = 185F. Little Timmy, as third license, gets an alarm on C01. He announces that the annunciator for Unit 1 motor breaker trip is lit.

What motors will cause this alarm?

In searching for the cause of this alarm, C01 OATC notices that P-32C which was previously running is no longer running and the white light is lit above its control switch. Regarding Tech specs only, is this an issue?

Unit specific CS, SI, CAF

Unit 1 MDAFW, Both SSG

SW A, B, and C

No, LCO 3.7.8 is still met with all other SW pumps available to supply cooling to required loads.

333

Define an expected alarm and give four examples of plant evolutions when an expected alarm would be expected.

“Expected” alarms are those alarms which occur in the normal course of plant operation.

-Starting / Stopping pumps

- Testing components / calibration

- Changing Plant Status

- Alarms identified to be expected prior to receipt.

487

What are the Generator Breaker trips on G07?

Control Switch to trip / breaker local trip button

Anti-motoring protection

Overload

Synch selector switch to OFF and motor breaker opens

487

Power supplies to W-30A, W-30B, W-21A, and W-21B

A's - 1B-42

B's - 2B-32

487

If actual blowdown flow is lower than what PPCS accounts for, is this a problem? Yes or no and why?

Yes this is a problem. Heat balance stuff that Justin doesn't necessarily understand happens. Effectively the calculated Rx power will be lower than actual Rx power and therefore non-conservative.

487

Little Timmy is trying to regain some composure and credibility with his crew. He's the 4th license and Unit 2 is performing Physics testing at approximately 3% Prx. He notices that AFD is outside the normal band and recommends that we restore AFD to normal band within the next 15 minutes in accordance with Tech Specs. Is he correct? Why or why not?

Unfortunately Little Timmy is incorrect. While performing Physics testing in Mode 2, AFD is outside the mode of applicability. AFD wouldn't come into play until Mode 1 > 15%
487

In a proportional controller, the term "offset" refers to the difference between what?

a. control point and setpoint

b. control point and proportional band

c. deadband and setpoint

d. deadband and proportional band

a. control point and setpoint

784

What 4 major breakers will open if I lose power to D11 and D12?

Unit 1 RTA and BYB

Unit 2 RTA and BYB

784

What is the expected CVCS lineup for a Dilution? (Not Alt Dilute)

CV-111 - Throttled

CV-110A - Shut

CV-110B - Shut

CV-110C - Open

784

Little Timmy is back on watch on Unit 1. We're cooling down the plant via condenser steam dumps in manual control. (CSD mode select switch is manual, 1HFC-484 is in automatic) When he attempts to lower the automatic setting, an electrical perturbation causes the complete loss of the Red Instrument bus. Does this loss affect the capability of the CSD system? If so, explain how to restore cooldown capability with the CSD's.

Red Instrument Power supplies PT-484, upon a loss of this power, CSD's will NOT operate in Manual-Auto control (Pressure control). CSD's will have to be controlled via Manual-Manual by taking 1HFC-484 to manual.

784

Per the TRM, what must be done if the LEFM is deemed not functional after the completion of the SR?

Initiate action to reduce Prx </= 1775 MWth immediately

784

What "two" radiation monitors will DIRECTLY cause a CVI?

1(2) RE-212

1(2) RE-305
