A request for incidental funding on behalf of a consumer with mental health and/or substance misuse diagnoses most typically for housing assistance, transportation, clothing or pharmaceutical needs and includes a plan to support the individual during this time of segue.

Adults or youth with a serious mental illness, substance use disorder, or co-occurring disorders who have high utilization of acute care services
Who are PRIORITY populations for Care Coordination?
A computerized, standardized assessment tool similar to common biopsychosocial assessment forms found in EHRs, but which integrates evidenced-based assessment instruments such as the Addiction Severity Index (ASI) within the assessment itself and provides a level of care recommendation.
What is ASAM Continuum?
Data about how an individual does over time in regards to employment, housing, benefits after having access to short-term incidental funding?
What is a Transitional Living Voucher Program Outcome?
Persons receiving services are empowered with resources and information to make informed decisions.
What is self-determination?
The # of days after an agency receives an assessment, evaluation, monitoring report from any stakeholder for any programs they provide that they have before it has to be submitted to SEFBHN
What is 10 days for any program an agency provides, regardless of funder?
Care Coordinators, provider staff, other provider agencies, consumer, consumer's "families", managed care organizations (i.e., WellCare, Sunshine, Beacon), ME staff
