create a SNow ticket using CWOW requisition not on file template a the end of the day, fill out the incident description. Write the incident number on the label with the associate tubes. Place tubes on the incident rack for the next shift to follow up.

How do you document a true duplicate tube?

At the end of your shift create one ticket in SNOW using the tracking duplicate tube. Scan all the tubes in the incident description section and update the requisition count in SNOW ticket.

After unpacking is complete, a specimen is spilled. Outline the full response: state the event classification, the tube type for cancellation, what must be included in the comments, and which two reporting mechanisms/forms must be used.

What is a “lab accident”, cancel tests using 5092 – RECOLLECT – LAB ERROR ACCIDENT, include SNow incident number in the comment (marked internal), and file a CWOW Specimen Issue (Lab Accident) incident in SNow plus create a SHIF form in SNow or TMSS.
