Locked and inaccessible to children, in their original, labeled containers and kept separate from food and dining surfaces.

Away from heat sources and inaccessible to children.

100

When is a child required to have their initial health exam (physical) and how often thereafter?

Within 15 days of admission and annually thereafter

100

What documentation must occur when a restrictive procedure occurs?

A health and safety screen, a safety plan, an incident data form, an emergency physical intervention progress note, and documentation should be included in the shift note for that client

200

Explain "all safe" and how it is used in your program.

All safe is a directive that can be given to clients if there is a crisis. All safe requires the clients to look away from the incident occurring, remain silent, take a seat, keep their hands to themselves, and remain in camera view. This is to protect clients and staff from additional incidents occurring and from clients reacting to the situation.

Kitchen and bathrooms

In their original containers, in an area or container that is locked, and under proper conditions of sanitation, temperature, moisture, and light.

300

Yes with court officials and attorneys. This should be facilitated by allowing the client a private space away from others where staff may have supervision of the client but cannot listen to the discussion.

300

Yes!

300

True or false: when fire drills are conducted, different exit routes should be utilized and drills should be conducted on different days of the week and at different times

True. Facilities Operations sends communication via email monthly to unit directors for the monthly drill. This communication includes the "location of the fire", the week to have the drill completed, and the shift the drill is to be completed on.

300

When is a child required to have their initial hearing and eye exam and how often thereafter?

Within 30 days of admission, however, these are completed within 15 days at Adelphoi when the physical is done. This is required annually thereafter and included in the annual physical.
300

A child who is deemed able to self-administer medication. If the child is unable, a trained staff member will administer the medication.

Immediately report to ChildLine. Notify your Unit Director afterward and discuss additional procedures with the Unit Director.

400

Why are temperature logs maintained on refrigerators and freezers?

Improperly stored food can create unsafe bacteria. It is required for cold food to be stored at or below 40 degrees. Frozen food must be kept at or below 0 degrees.

400

What do you do if an emergency evacuation must occur within your program?

Depending on the type of evacuation, the group will be notified, staff must prepare to leave for the primary or secondary evacuation location, medications and emergency supplies must be packed, and the evacuation should occur. The Unit Director, Program Director, county workers, and family members will need to be notified.

A child has the right to send and receive mail. Outgoing mail may not be read or opened by staff persons. Incoming mail from federal, state, or county officials may not be opened or read by staff. Other incoming mail cannot be opened or read unless there is reasonable suspicion for contraband. If this is suspected, mail should be opened in the presence of the child.
