List one reason it is important for supervisors to fill out the supervisor walkthrough form (100 points each correct answer, multiple allowed)

-Documentation to protect from liability

-Ensure your team members' safety

-Identify if your team member is aware of the safety hazards concerned with the task. If they don't, a learning opportunity and avoiding preventable incidents with training

-Identify areas that may need more attention (housekeeping, organization, work methods, shortcuts, improvements, etc.)

Junk removal picked up the old scrubber unit from the back alley and caused some NMC slurry to spill onto the ground. It was unknown how exactly this amount of slurry spilled, whether the spill occurred before the junk pickup, or how much hazardous chemical is still inside the scrubber tank, which is now gone.

What could have prevented this incident?

Cleaning the old scrubber equipment thoroughly before disposing of it.

An operator doing a synthesis was using a NOx detector to measure the atmosphere near the reactor, setting off the NO2 alarm falsely.

Name one corrective action to prevent false alarms or improve this procedure (in terms of safety).

-Ordering separate NOx sensors to use for reaction monitoring

-Communication to team about the correct use of the NOx sensor

-Revising the SOP to specify sensor use.

200

What 2 things should you not do if you begin hydroplaning while driving to work?

1. Hit the brakes

2. (over)correct the wheel

would also accept: panic!

300

What are the 3 things you must do when you receive a new chemical (in terms of SDS) before it is brought to the lab and used?

1. Print 4 copies of the SDS and put one in each binder

2. Upload one electronic copy to the N-drive SDS folder

3. Update the master database SDS List.xlsx

A fizzing sound in 101A warehouse and a trail of clear liquid leading to the H2O2 barrel. The pump was left on top of the barrel and was dripping. Employees gathered the pilot/scale up team and demonstrated how to clean the spill and communicated safety issue to them.

List two corrective actions resulting from this incident

-Create SOP for H2O2 transfer from large containers to small containers and clean up of pump

-Place H2O2 drum on spill containment skid

-Retrain staff who use H2O2 on proper transfer and clean up procedures

-Making a containment pipe for the pump/ or order a plastic siphon pump to be placed permanently in the drum

300

Who's responsibility is it to tell someone to put on safety glasses when walking through a lab? (even if they are not on your team, are a higher-up)

Everyone! It is your right but also your responsibility

2. Their supervisor (unless they are a supervisor)

3. One member per each team that will be partaking in the activity for the JHA

4. One member of the JHSC

400

An operator cut his hand while using a crowbar to separate the panels on a large delivery crate.

What is the correct order of steps to follow in the days and weeks after if it was your hand that was cut?

1. Receive first aid

2. Inform supervisor & JHSC to do an investigation (take photos, interview witnesses)

3. Fill out incident report form

4. Inform supervisor that incident report was filled out and follow-up with them to fill out/complete action items.
