Family Advocates ensures the health forms necessary to complete the child’s file are completely filled with proper signatures by assisting parents in completing the General Screening Consent Form and the Health History Form. A Medication Administration Form is distributed for any child who will be on medication while at school. Up-to-date and current immunizations and a TB skin test (result must be within one year) or TB screening clearance is required by this time for ‘enrolled’ status.

Family Advocates offer parents opportunities to develop and implement individualized Family Partnership Agreements that describe family goals, responsibilities, timetables and strategies for achieving these goals as well as progress in achieving them. In home-based program options, this Agreement must include the above information as well as the specific roles of parents in home visits and group socialization activities

Teachers and Case Managers (Home Visitors) must complete this to conduct developmental screenings for cognitive, physical, social/emotional and language development within 45 days from child's first day or entry date in the program.

The classroom team (Teachers) and Case Managers (Home Visitors) must meet with the Mental Health Interventionist to discuss and develop a treatment plan for each child's social and emotional health and development every other week.

The Family Advocates receives all of the required paperwork for the child; then creates and organizes the child's/family files; places the health forms and face sheets for each section (i.e. contact log, education, disabilities, etc); and gives to the site director who will review the Individual Infant Needs and Service Plan, child's health history and parent survey with the teacher before the child steps into the classroom.

Parents are invited and encouraged to attend and/or become members and get involved in Center Committees, Health and Nutrition Advisory Committees and Policy Councils including male/father involvement in the program.

Teachers must wash their hands first, put on a glove while cleaning and removing diapers; and must remove the dirty gloves before putting on clean clothes and diaper on a child.

Teachers and Case Managers (Home Visitors) must use this to complete on-going developmental assessments initially within 60 days from entry date and every 4 months thereafter to assess children's development and growth.

Parent participation in the program is essential and best practice. Grantee must establish Policy Council and Parent Committee Structure through which parents can participate in policy making or in other decisions about the program. at least 51 percent of PC members must be the parents of currently enrolled children.
