WHAT IS RAC AND THE 5 CODES THAT GOES WITH IT?
Risk Assessment Code (RAC) is an expression of risk associated with a hazard that combines it severity and probability into a single number which can be used to help determine hazard abatement priorities. 1 - Critical 2 - Serious 3 - Moderate 4 - Minor 5 - Negligible

DESCRIBE THE EFFECTS AND TREATMENT OF THE FOLLOWING TEMPERATURE RELATED INJURIES;
HYPOTHERMIA - Occurs when the body is exposed to subnormal water temperatures. To overcome hypothermia, minimize heat loss from the head, neck, sides and groin. Raise body out of the water as much as possible. FROSTBITE - Usually affects the hands, face, or feet and it most often occurs on windy, very cold days. Treat the affected part immediately by placing it in contact with a warm part of your body. Cover it with your hand or put frozen fingers inside of your clothing. HEAT STRESS - Excessive sweating may result in painful cramps in the muscles, of the abdomen, legs and arms. Heat cramps result from drinking ice water or other cold drinks either too quickly or in too large of a quantity after exercise. Treatment move victim to a cool place. Give plenty of water. HEAT EXHAUSTION - Caused by working/exercising in hot environments. This causes a serious disruption of blood flow to the brain, heart and lungs causing victims to experience weakness, dizziness, headache, nausea and loss of appetite. Treatment includes; Moving victim to cool area. Loosen clothing. Apply cool wet cloth to victim's head, axilla, groin and ankles. HEAT STROKE - Victim may experience nausea, dizziness, or weakness. Breathing will be deep and rapid at first, later shallow and almost absent. Victim is usually flushed, very dry and very hot. The pupils will be constricted and the pulse fast and strong. For treatment, immediately douse the body in cold water or apply wet cold towels to the whole body. Move victim to the coldest place available. Maintain an open airway. Place victim on their back, with the head and shoulders slightly raised. If conscious give victim water to drink.

What is the 5 step process of ORM?
1) Identify Hazards= a condition with the potential to negatively impact mission accomplishment or cause injury, death, or property damage. 2) Assess the Hazards= to determine the associated degree of risk in terms of probability and severity. 3) Make Risk Decisions= Develop risk control options starting with the most serious risk first and select controls that will reduce the risk to a minimum consistent with mission accomplishment. 4) Implement Controls= Measures that can be utilized to eliminate hazards or reduce the degree of risk. 5) Supervise= Conduct follow-up evaluations of the controls to ensure they remain in place and have the desired effect.

WHAT ARE THE TIMELINES AND MEANS FOR FILLING MISHAP INVESTIGATION REPORTS?
TIMELINES?? Means - record keeping and reporting is vital to provide safety information to the Department of the Navy and Department of Defense. These records and reports are required by federal law and provide information to identify unsafe acts and conditions, and apply corrective measures. All electronic entries are transmitted to Command Naval Safety Center in put into the mishap database.

DESCRIBE THE SYMPTOMS AND TREATMENT FOR SHOCK
Symptoms; - Pulse is weak and rapid - Shallow, rapid and irregular breathing - Cold skin - Sweating more noticeable - Very pale skin and may be bluish or reddish color Treatments; - Insulate the victim from the cold and maintain body temp - Elevate legs 8-10inchs

DEFINE HERO, HERP, HERF.
HERO - Hazards of Electromagnetic Radiation to Ordnance is the program concerned with prevention of accidental ignition of electrically initiated devices in ordnance due to RF electromagnetic fields. HERP - Hazards of Electromagnetic Radiation to Personnel is the danger of producing harmful biological effects in humans by exposure to radio frequency electromagnetic fields. HERF - Hazards of Electromagnetic Radiation to Fuels is the danger of igniting volatile combustibles by spark ignition due to radio frequency electromagnetic fields of sufficient intensity.

WHAT IS HAZARD SEVERITY AND THE 4 CATEGORIES OF IT?
Hazard Severity is an assessment of the worst credible consequence that can occur as a result of a hazard. Severity is defined by potential degree of injury, illness, property damage, loss of assets or effect of the mission. CAT I - Loss of the ability to accomplish the mission. Death or permanent total disability. Loss of a mission -critical system or equipment. Major facility damage. Severe environmental damage. Mission-critical security failure. Unacceptable collateral damage. CAT II - Significant degraded mission capability or unit readiness. Permanent partial disability or severe injury to illness. Extensive damage to equipment or systems. Significant damage to property or the environment. Security failure. Significant collateral damage. CAT III - Degraded mission capability or unit readiness. Minor damage to equipment, systems, property, or the environment. Minor injury or illness. CAT IV - Little or no adverse impact on mission capability or unit readiness. Minimal threat to personnel, safety, or health. Slight equipment or systems damage, but fully functional and serviceable. Little or no property or environmental damage.

HOW DO YOU TREAT ELECTRICAL SHOCK?
Rescue- if possible secure the power and try to remove the person from the area. Use a non-conductive objective to remove the person if the power cannot be secured. Treatment- Administer artificial ventilation immediately if the electrical shocked has caused breathing to stop. -Check victim's pulse, if no pulse immediately administer CPR.

WHAT IS MISHAP PROBABILITY AND DESCRIBE THE 4 SUBCATEGORIES OF MISHAP PROBABILITY.
Assessing the Probability is the likelihood that a potential consequence may occur as a result of a hazard and is defined by assessment of such factors as location, exposure, affected population, experience, or previously established statistical information. CAT A - Likely to occur , immediately or withing a short period of time. CAT B - Probably will occur in time. Expected to occur several times to an individual item or person, or frequently over a service life for an inventory of items or group. CAT C - May occur in time. Can reasonably be expected to occur some time to an individual item or person; or several times over a service of life for an inventory of items. CAT D - Unlikely to occur, but not impossible.
