When choosing the Case ID for a Memo, do you choose the open adoption Case ID or the open clinical Case ID?

Clinical.

100

Where on Zoom Contact Center can you find the police numbers?

You can find them under "Addresses"

100

All Institutional reports go to the local DCF office where the child's case is open?  True or False?

False.  Institutional reports go to the DCF office where the Institution is located.

100

How long can you keep an Alert if DCF calls about a missing child and does not have a placement if found.

One business night or an entire weekend.  See Process Maps.

100

Are adults listed on the person’s page if we do not have their names?

No.  We would put an adult on the Person's Page as an "Unknown" only on Institutional report.  We do not put "Unknown Mother, "Unknown Father", etc on 51As. This will create unwanted PIDs.

200

Can we take a Memo on a child who has an open case, but is not in DCF custody?

Yes. We are able to take memos on open cases, even if the child is not in custody.  We do not take memos on cases that have been closed.

200

What do you click on to make a conference call?

You click on "Add Call"

200

Where do you put the child's parents' names (if given) on an Institutional report?

The child's parents' names go in the Clarifying Paragraph, not on the Person's Page.

200

Why is it important to always check the Alerts?

DCF may have left the EL information and directions regarding one of their cases that the EL needs to follow.

200

You are going on break or leaving your shift, but you have a report pending with the police or OCS.  What do you do?

The report needs to be sent over for approval as a non-er with a note at the top of the narrative stating this the report is pending with the police or the OCS. Do not leave the report on your tab because the Sups and CMs will not see it. Notify the Sup that you are doing this before leaving.

300

Do we have to call an OCS for every Memo that we take?

No.  Not every memo (just like with 51As) requires an OCS call.

300

What type of transfer ("warm" or "direct") allows you to speak to the person first before either transferring or conferencing them in

"Warm" transfer

300

Record Reviews do not need to be done on Institutional Reports.  True or False?

False.  The EL will still do a Record Review on every Institutional Report.  Any history on the institutional will also need to be noted in the Record Review.

300

How long can you keep an alert if DCF has a placement for a missing child, if found?

If a placement is willing to keep a bed open for the child for 2 weeks, that is the longest the EL can hold the Alert.  See Process Maps.

300

Name some information that should go in the Clarifying Paragraph.

Document who lives in the home. Document if there are any non-reported children (list their names).  Document any information the reporter was unable to provide in the demographic section.  If there is an open case, note any differences in demographic information. Note if the family speaks another language other than English.

400

What does "Placement Risk" mean on a Memo?

The child may have, for example,  sexualized behavior, be allergic to nuts, cats, bees, or be in a wheelchair, etc. This information will help if the child needs to be placed by the EL.

400

What "not ready" disposition do you use for your one hour break?

Meal

400

The reporter is filing physical abuse on one foster child by another foster child in the home. The reporter is not filing against the foster home.  Is this a 51A or an Institutional report?

Due to this occurring in a foster home, this would be an Institutional report.

400

An alert is filed on a child in the mother's custody.  The mother may call saying that she wants the child removed from her home.  The worker, who told the mother what to do when she calls the EL,  may alert the Emergency Line to do WHAT if the child needs to be removed and placed elsewhere?

The EL will need to take a 51A in order to move forward with this due to the child not being in DCF custody. We cannot go out and remove a child who is not in DCF custody.

400

The Contributing and High Risk Factor questions should be in a different format than the rest of the narrative.  True or False?

False.  The HRF questions should remain in paragraph form, in full sentences, and include all the information the reporter provides.

500

What is a Request for Response that comes in via DCF?

This is when the DCF Office takes a report, most likely, later in the day, and wants the EL to respond.  DCF is closing and believe that this report should turn in to an ER.

500

Where do you check for supervisor assignments and breaks at the start of your shift?

EL All Staff chat/channel

500

If someone files on the biological child of an active foster parent, is this an Institutional report or a 51A?

This is an Institutional report because the biological parent is a foster parent.

500

Can the Emergency Line take an Alert from a worker on a Saturday night?

Yes. Alerts don't necessarily have to come in during the weekday; sometimes the workers are doing work on the weekend and need to file an alert.

500

A Plan of Safe Care is a document created to ensure the safety and well-being of infants and their families who are affected by substance use, particularly prenatal substance exposure.  What allegation requires the Emergency Line to ask if a Plan of Safe Care was created?

Substance Exposed Newborn.

600

With a Request for a Response, who do we need permission from (from DCF) in order to process this request?

APM (Area Program Manager) or AD (Area Director)

600

Can you call an OCS from the "phone" app?

No, the "Phone app" is for internal calls only.
600

A reporter is filing physical abuse on a child in a residential home on another child in the residential home.  The reporter is not filing on the home.  Is this a 51A or an Institution?

This is a 51A, not an Institutional report because the reporter is not filing against the residential home.  This would be different if it was in a foster home (it would be an institution).

600

Can an Alert be filed by a residential program?

No.  Alerts are only filed by DCF.

600

The father is involved in a case as a child.  The father is also involved in another case with another family unit.  The reported child and the mother are not in either case.  Which case do you pick, if one?

You do not pick any of those cases.  You will create a new case.  The new family unit (child, mother, and father) are not involved with the father's case as a child and not involved with the father's case with another family unit.
