Provide the Electric Trouble Dispatcher with the facility the employee will be transported to, who will be following the ambulance, and a contact number for the following employee.

LADWP vehicle breakdowns, flat tires, and minor accidents involving LADWP vehicles where none of the involved parties suffer injuries.
