A 2 year old is brought to his pediatrician because his mother is concerned about his eyes. For the last week, she has noticed that his eyes seem to be moving around rapidly, randomly, and erratically in all directions and does not stop even when he is focusing on something. Yesterday, she also noted his arms and legs seems to jerk sporadically. He has otherwise been well with no recent illnesses. What should you be concerned about?
What is This patient is exhibiting signs of opsoclonus myoclonus syndrome. 40-50% of patients who presents with this will be diagnosed with neuroblastoma [though only 2-3% of neuroblastoma patients will exhibit these signs]. Evaluate with homovanillic and vanillymandelic acid [catecholamines] in the urine [as 90% of neurobastoma cells will secrete hormones that get broken down into these products], MIBG scan, CT/MRI, U/S, or PET scan can also be performed.
