To facilitate residents achieving the scholarship and project management goals of the residency program, this committee shall provide a research certificate series to all residents in the form of regular lectures and periodic assessments of project progress.
What is the RTF?
Leave time or professional meeting time may be used in lieu of PPE and on-call expectations
Failure to pass Illinois pharmacy licensure exams by this date will result in termination of the resident contract and release of the resident from the residency program.
This is the
numeric rating scale used in the UCM Summative Resident Assessment conducted at
the end of each learning experience.
What is 1-5? (PH05-04, PharmAc)
One (1) – Needs Improvement, consistently performs below expectations for the learning experience
Two (2) – Approaches, but does not fully meet expectations of the learning experience, or progressing slower than anticipated for a resident at this time of the residency year
Three (3) – Consistently meets the expectations and embodies the values of the learning experience
Four (4) – Exceeds the standards or expectations of the learning experience, or progressing faster than anticipated for a resident at this time of the residency year
Five (5) – Provides an example for other residents, or performs at a level beyond that expect of a resident
This is the date that final manuscripts due to project mentor & RPD.
What is March 31st (PH05-11 APX A)
A group consisting of a minimum of three preceptors (1 of which is a member of the REC) responsible for scheduling seminars, recording attendance, collecting evaluations, and delivery of evaluations to the presenter.
What is the Grand Rounds Task Force?
External moonlighting is strictly prohibited and may be considered a violation of his/her residency agreement.
The DGPE and RPD shall provide match applicants with the details of their appointment in writing. This appointment agreement must be sent to all matched applicants and be postmarked no later than this many days following receipt of Match results.
The resident must receive a score of this or greater on 100% of all required residency goals by the end of the fourth quarter.

What is Tuesday, March 27th and Wednesday, March 28th
The residency committee responsible for tracking residents post graduation from the residency program.
For residents on an operational PPE block, PTO time may not be requested during these blocks.
Learning experiences shall be evaluated by residents within
this many days of completion of the learning experience.
This shall be completed by incoming residents and returned to the RPD prior to beginning the residency program.
To be considered for a PGY2 position via the Early Commit process, a formal letter of interest shall be provided by this date to the PGY2 RPD, copied to the PGY1 RPD, DGPE, and Chief Pharmacy Resident
What is the second Friday of November? (PH05-03)
The designated resident in charge of documenting events of the residency through written notation and photographs.
The final set of Grand Rounds slides are due to GRTF this
many days before the scheduled presentation.
The oversight committee responsible for input and direction regarding residency program activities. The committee shall be chaired by the DGPE and shall consist of pharmacy preceptors appointed by the DGPE, including at a minimum all RPDs as well as the Chief Pharmacy Resident.
What is the Residency Executive Committee (PH05-01)
This is the residency program year that our modern day residency programs were launched at UCM.
What is 2009-2010?
The Chief Pharmacy Resident should be appointed by this month of his/her PGY1 residency year in order to allow for adequate pass off from the current Chief Pharmacy Resident for the upcoming year.
