What three parts must be included when creating a new report from a closed report?

Will the person i am reporting get a copy?
That information is determined by your company and is not provided to us. If you have any concerns about the report being released to particular individuals, we can document that in the report.
Can I lose my job for filing this report?
Our clients discourage retaliation in any form. If you feel that you have been retaliated against for filing this report, you are encouraged to call us back so we can add that to your report.
Should I remain anonymous or give my name?
If you would like to provide your name and contact information, the company will be able to contact you directly about the report. If you do not feel comfortable providing your name, we can file your report anonymously if your company allows it. In that case, we would refer to you as “the caller” throughout the report, and we will not disclose your gender. We will exchange a report key and password at the end of the call that will allow you to follow-up on the report.
