What is a Strain?
Sloping will be 1 foot vertical, 1.5 feet horizontal (34˚). Sloping/Shoring for excavations greater than 20 feet deep shall be designed by a registered P.E.. Type “C” soil is not to be benched.

What is proper lifting technique?
Cave-ins, Asphyxiation due to lack of oxygen, Inhalation of toxic materials, Collapse due to machinery movement near the edge of the excavation, Fire, Water accumulation, Access/Egress, Falls.
What are the various hazards associated with trenching and excavation?
All work should be done de-energized. Only B&M qualified team members are allowed to work on energized circuits, after obtaining a permit and the proper PPE

What is STKY?
A person who locks out/tags out machines or equipment in order to perform service or maintenance on that machine or equipment. An affected employee becomes an authorized employee when that employee's duties include performing service or maintenance covered under this section
What is an Authorized Employee?
With heavy loads or repetitive tasks, whenever possible, use mechanical aids such as forklifts, cranes, chain hoists, carts, etc.
What are mechanical aids?
A ladder or ramp must be present in trenches & excavations that are 4 feet or greater in depth. Ladders or ramps must be installed every 25 feet for access and egress. (see diagram below) The ladder shall extend 3 feet above the excavation. Spoil shall be placed 2 feet or more back from the edge of the trench or excavation.
Wat are 4 foot depth requirements?
Keeping a site clean and clear of debris improves worker safety by creating fewer trip hazards
What is housekeeping?
Identify hazards associated with each task, eliminate the hazards from the work task, ensure that safety is the first thing discussed prior to the start of the shift,
What is the Purpose of the JSA?
Specific practices and procedures to safeguard employees from the unexpected energization or startup of machinery and equipment, or the release of hazardous energy during service or maintenance activities
What is Lock-out/Tagout?
Communicate and plan the move, determining the lift path and any obstacles. Both individuals should bend at the knees (not the waist), keeping their backs straight and lifting with their legs. Lift and move in sync, maintaining communication throughout
What is a TEAM lift?
Along with the 4 feet depth requirements listed above, additional measures are required prior to entry. Sloping or protective shielding (trench box) shall be in place. Protective systems must be able to resist all expected loads.
What are 5 foot depth or greater requirements?
Supervisors should schedule work around other contractors to avoid unforeseen hazards
What are other Trades/Contractors?
STAR(Stop, Think, Act, Review), Self check, Peer check, Stop/timeout, etc.
What are HOP tools? (Human and Organizational Performance Tools)
A device containing multiple keys inside it with locks on the outside of it from every worker associated with the locked out equipment.
What is a lock box?
Hard hats, work boots with proper ankle support, knee pads or kneel pads (when needed), and gloves. Hi-Viz may be required on you’re the job based on vehicle or equipment exposure
What is PPE?
Daily before work starts and as needed if conditions change. o After rainstorms, high winds, or other occurrence which may increase hazards. o When you can reasonably anticipate an employee will be exposed to hazards
What are daily pre-entry inspections?
Specifically identify anywhere a part of the body can get caught between two objects
