No, but we can provide resources and information about the IEP process, school related resources, and community supports that can help families better navigate the special education system.
What does the Family Resource Team need from a provider to complete a Ben’s Fund request?
Specify what family has requested through Ben’s Fund (swimming lesson, etc) and sign letter on Epic.
Where would you direct a family that has questions about accessing diapers through Medicaid after a referral has already been placed?
Diapers PE (available in multiple languages).
Does the Family Resources Team maintain a list of providers?
No, but providers can direct families to AS360 and/or families can check in with their insurance plan.
What does Family Resource Team need from provider to support family with ABA navigation?
An ABA order in your note
Where can providers find common family resources and Patient Education handouts?
Autism Center Patient and Family Education Website, the Patient Education Database via CHILD, and team rooms.
Does the Family Resources Team support with filling out Guardianship paperwork?
No - but we can provide PE handouts, link to classes taught by MLP, and connecting to community resources such as Arc of King County.
Where can you check requests from the Family Resource Team?
Your Epic InBasket (letter queue, clinical coordination, etc.)
