H&P present and dated within 24 hours of admission or if same day admit for a procedure, H&P must be dated and timed prior to procedure start time. If H&P was completed within past 30 days, an updated note is acceptable within 24 hours after admission but prior to procedure requiring anesthesia.

Didn't bring it: tell them to bring it with them next time to be loaded into the system. If there is concern about DRN status - escalate to GI provider, Anesthesia, and leader to get it entered into EPIC as an order and discuss plan of care for procedure.

90 days (3 months) after opening - whomever opened it.

Remove the employee from the patient area. Escalate to manager (no message or text), notify risk management
