How can an applicant complete their EOI?
Any applicant can complete their EOI online by using their digital device through our secure portal.
They can also obtain a paper EOI by printing a paper copy through the portal or calling customer care to have a paper copy

Life insurance pays the designated amounts to the beneficiaries on file upon the death of the insured.

Up to 31 days from date of hire new eligible employees are able to enroll in benefits

What happens if the EE does not want to complete their EOI?
If an employee no longer wants the coverage or the employee/spouse applicant does not wish to complete EOI we can close their file.
They must also notify their HR or Benefits center regarding this decision, so their benefit elections can be updated.
They can choose to have the file closed at any point in the process, either before or after submitting their EOI.
If an application is closed this will not affect any existing coverage only the new requested coverage/increase in coverage.

How does MU determine if an applicant is approved or declined?
MU guidelines assign a maximum ‘’debit’’ threshold for each coverage type (STD has the lowest threshold, then LTD and Life has the highest threshold)
MU will assign debits for each bmi, tobacco use, height/weight, medical surgical procedures, medical conditions and medications
If total exceeds threshold coverage will be declined and if below threshold coverage will be approved

SIB pays a percentage of the Employees salary as of last day worked (LDW) to the beneficiaries upon the death of the insured.

What happens if there is not enough info for Medical Underwriting (MU) to make a decision?
If the MU determines there is not enough information provided on the initial application to make a decision we may request addition information
We may contact your physician for medical records, office visit notes, test results or lab results. We may also require your doctor to submit an Attending Physician Statement (APS) with these records
We may also request you have a paramedical exam performed by a doctor other than your PCP

Accidental Death and Dismemberment (ADD): Additional coverage if the cause of death is due to an accident. Also pays set percentage of coverage in the event of dismemberment.
