EE was attempting remove slack of chain when he caught his finger between bucket and chain.
EE was needle scaling when the weldment fell and caught finger between two objects.
Unsafe act - EE had finger in danger zone / pinch point
EE was welding w/o a heat shield. EE was holding hand close to end of lead and felt heat building up but continued to weld.
Unsafe Act - EE didn't have proper PPE and hand placement too close to heat source
EE was cleaning turnings out of manual lathe. Grabbed ball of shavings resulting in laceration to thumb.

EE was using hammer to force pin into bucket bushing. Continued blows resulted in laceration of right middle finger.

EE using pocket knife to cut hose and barley misses cutting his finger.

EE was attempting to knock off slag with chipping hammer when he scraped finger resulting in laceration.

EE was lifting magnet with crane. As he was lifting his index finger got caught between the chain and magnet

EE was in the process of tacking down a stamp and was pushing stamp down with hand resulting in contact burn.

EE was tacking brace bar to panel. As crane was moved up & over to position hitch plain, crane chain popped tack on brace bar striking EE’s left pinky finger.
Unsafe Act - Lack of written SOP and inconsistency in training
EE attempted to brush wire off weld with gloved hand. Wire punctured EE's finger.
Unsafe act - Using hand versus tool
EE was attempting to place plate dog on the sheet steel and reached under sheet to move wooden block. Plate dog was knocked off the sheet steel, smashing finger between steel & wooden block.
Unsafe Act - Placed hand in danger zone / Not following SOP
EE was writing inventory cards, moved a cylinder bracket that was nestled inside a thumb, bracket slipped out of his hand pinching left index finger between thumb and cylinder
Unsafe Act - EE not wearing cut resistant gloves while handling parts. Parts nested inside thumb body.
