A document that is received by the parent and provider, once the family has been approved through CCA.

How often is a licensed provider to renew with CCA?
Renewal coincides with licensure expiration. When a licensed provider renews with CCL (Child Care Licensing) they also need to submit renewal paperwork to CCA. These are seperate agencies!

1. Working 2. Seeking Work - MAX OF 80 HOURS PER CALENDAR YEAR. This may be issued in 16 PT Days or 8 FT Days of Attendance ONLY. 3. Attending Educational Courses and/or Training. * All activities must be verified by family. * If a 2 parent household, BOTH must be in an activity.

What is a "School Age Blurb"?
The School Age Blurb will be on the bottom of authorization documents if a child is attending school. This blurb reads, "Payment to be made at a FT Daily Rate minus a PT Daily Rate, not to exceed a FT or PT Monthly Enrollment Rate."

How long does a provider have to submit a billing report form?
90 Days except for the month of May and June, due to fiscal year turnover.

What are various ways to submit Pass I/II/III billings to Fairbanks CCA?

When may a family owe more to a provider than the required co-pay amount given by CCA?
The Facility Rate is more than the state maximum rate. *This difference will not be looked at when a provider reports non-payment.

What is the difference between an attendance and enrollment authorization?
1. Attendance is based on family activity schedule and results in the child being in facility 16 days or less per month. 2. Enrollment is based on family activity schedule and results in the child being in facility for 17 days or more per month.

What are reasons why my bill was rejected?
1. Missing PVN, ICCIS Number, Signature, Totals, Incorrect Name for Children or Parent. 2. No Authorization for child. 3. No record of attendance marked on billing form. 4. Ineligible hand writing. 5. Provider used white out or pencil.

When a rate change is submitted, what will be the 1st effective date?
30 Days +1st of the month. For EX: A rate change is submitted 11/5/2012. (30 Days from 11/5/2012 is 12/5/2012. 1st of following month is 1/1/2013). Effective date of rate change would be 1/1/2013.

Why can't CCA tell me anything about my families?
Per Department of Health and Social Services, along with HIPAA, Child Care Assistance is unable to provide any information to a provider, without written consent from the parent. If written consent ( AKA Release of Information) is granted to provider and submitted, CCA and Provider may communicate based on family eligibility and case information only. **CCA is not allowed to produce a form for this, and it must be completed between parent and provider only.

Authorizations can't be changed for the ______ month we are in.
1. Current Why? CCA Policy and Procedure states we are unable to change current month authorization. We are required to give a 10 day notice to families, when it raises co-pay or decreases units of care. Other reports of change will take effect the 1st of the following month EXCEPT with Increase in care that is reported by parent. This will be forwarded onto billing.

What is the quickest way for a family to receive approval?
The quickest way for a family to move through the application to determination process would be; 1. Submit ALL information at time of application. This may include; Verification of birth for children, Government Issued ID, all income received, and training/education documents (if applicable). 2. Schedule an interview, timely. This is the parent responsibility. CCA does not contact parents to schedule an interview, except by mail. 3. Parent must be able to give a provider name and start date before processing application to approval/denial. *An authorization is not possible before a family starts at a facility.

Who completes Pass I authorizations?
Pass I authorizations are processed through CCPO in Anchorage. Parents receiving Pass I benefits must contact Public Assistance Caseworker for assistance.

A parent does not give a 2 week notice and bails. What am I allowed to bill?
If the child has a enrollment contract, and has attended at least 1 day in that month...you can bill for the entire enrollment amount if you choose. If the child has a attendance contract, provider will only be paid for days the child was in attendance. The family will not be authorized to attend another facility for 2 weeks from the report of change.
