A family is placed on Level CO status when a scheduled visit results in a missed visit followed by another unsuccessful attempt to reschedule (i.e., a parent cannot be located or a rescheduled visit is also missed).

Transition plan is created when a family is exiting services. A transfer plan is created when we are transferring the family to another HFA program.

What could be some CO methods?

  • warm telephone calls focused on the family’s well being
  • creative and upbeat notes which encourage parents to want to participate
  • drop-by visits (exercising safety) and leaving a card when families are not home
  • texting brief messages to let a parent know you are thinking about them
  • anchoring conversations based on family’s interests
  • encouraging self-care practices

What is the difference between re-opening a case and re-enrollment? Will a FROG be needed for either?

Re-opening a case is opening a case with the same target child if their closure was within 6 months or less. A FROG is not needed for re-opening. Re-enrollment is the same family enrolling with a new target child if they meet the criteria for HV services and have a positive FROG.

What does a Transition Plan need to include?

a. reason for closure, 

b. the date of the initial discussion with the family regarding closure, or date family declined a transition plan,

c. the planned closure date, 

d. identified resources/services needed by the family and 

e. an outline of steps needed to obtain any identified resources or services

f. Follow-up with identified resources by the site and/or family before closure to determine availability of services and assist with the transition.

When do you need to complete a depression screen pre- and postnatally? What is the documentation process for referrals for those who screen high?

Depression screen is done at the second or third visit for prenatal moms and postnatally within three months of the baby's birth (or within three months of enrollment if enrolled postnatally). If screen high, must document in a note you provided a referral AND the outcome of the referral.

When does CO conclude?

Level CO/Creative Outreach status is concluded prior to three months (whether 3 consecutive months or for a cumulative three-month period over six consecutive months).
