  • The family and youth share their lived experiences, strengths, cultural background, needs, and vision for the future.
  • The Wraparound Facilitator supports this process by guiding conversations, asking thoughtful questions, and ensuring the information is captured accurately and respectfully.
  • Support partners (such as the Family Support Partner and Youth Support Partner) help ensure the family's and youth's voices are fully represented and understood.

So, while the facilitator compiles and organizes the SNCD, the content is determined by the family and youth, making them the primary source of the information.
