  • The family cannot discharge voluntarily before completing the purposeful transition required by HFW.
  • DSS must be the entity that approves, initiates, or authorizes early termination.
  • The HFW team must continue to engage the family while communicating with DSS about:
    • Progress
    • Barriers
    • Safety concerns
    • Engagement challenges
    • Readiness for transition

Even when families are mandated, HFW still operates from the principles of family voice and choice, but choice does not override the legal requirement to participate.

Why?

  • Implementation Phase:
    During this phase, the team practices collaboration, builds skills, and increases family voice and choice. Families begin taking on more leadership as they gain confidence and mastery of the process.

  • Transition Phase:
    This phase begins when the family is able to sustain progress independently, demonstrate strong natural support networks, and lead their own team structure without heavy reliance on the facilitator.

So, when a family is facilitating their own CFTM, it’s a strong indicator they are transitioning toward graduation and long‑term independence.
