A recent work document states:  Employees have 10 sick days per year.  Any absence longer than 5 days will require a doctor's note.  Sick days are non-transferrable and do not roll over into the following year.  To claim a sick day, the employee must contact their immediate supervisor 2 hours prior to their shift.  When filling out a vacation request form online, be sure to include your name, date(s) to be absent, job position, and supervisor's name.  A copy should be submitted to HR and your supervisor.  This form should be submitted one week prior to the dates requested.  Once submitted dates are final, unless the employee becomes sick or has a death in the family.  In this situation, the employee has 48 hours to petition their request.  Failure to follow these steps will result in disapproval of any request to reclaim a vacation day.
