  • Socializations are facilitated by Home Visitors using the PIWI (Parents Interacting with Infants) early intervention model and DOT model) and the two curriculums we use

  • At the beginning of the 3rd trimester of pregnancy (if this falls within the 45 days do not complete twice)

  • Any time a mother is experiencing signs of depression related to postpartum; If Home Visitor is unsure they should bring concerns or observations to monthly case management meetings to Home Based Manager to discuss options

  • Home Visitor will use the maternal health depression follow up form to sent to HBM
  • Any EPDS with a score >12 will result in providing the Mother with a referral for mental health support.
  • Any answer other than Never on question 10
  • HBM will meet with clinical team within 7 days and provide the form back to the Home Visitor with recommendations that she will bring back to the mother

What is the process in creating goals with families and how many should each parent have? How do we support them in that process. Where do we document?

At least 1 goal best 2 if possible and we support them by using different tools such as pre-assessment, scales and ladders and their input. We create achievable goals with step by step responsibilities and document it as an event under self sufficiency goal
